Gluconormet

Gluconormet (Glimepiride / Metformin HCl) by AGP tablets 2 mg / 500 mg is an oral antidiabetic medication, containing 30 tablets. It is used for the treatment of type 2 diabetes.

Package Contains: Each Gluconormet package contains 30 tablets of 2mg / 500 mg & a leaflet.

Composition: Each tablet contains:  Gluconormet

Glimepiride ( USP specs ) 2mg Metformin HCl ( USP specs ) 500mg

Dosage: 1-2 tablets daily or as directed by the Physician.

Uses / Indications: Gluconormet Tablets are indicated in the treatment of patients with type 2 diabetes mellitus when diet, exercise, and single agent do not result in adequate glycemic control.

Dosage & Administration: Gluconormet should be given once daily with a full meal, preferably in the evening. Therapy should be started with gluconamate (glimepiride 1 mg & metformin 500mg ) one tablet daily. Depending upon the glycemic control, the dose may be titrated to 2 tablets once daily.

Alternatively, the therapy may be started with gluconamate ( Glimepiride  2mg / Metformin HCl 500mg ) one tablet daily. Depending upon the glycemic control, the dose may be titrated to 2 tablets once daily. The maximum daily dose of Glimepiride is 8 mg & metformin is 2000mg.

Side Effects / Adverse Reactions:

  • Glimepiride: The adverse reactions encountered with Glimepiride are vomiting, gastrointestinal pain, diarrhea, and allergic skin reactions. Leukopenia, agranulocytosis, thrombocytopenia, Hemolytic anemia, aplastic anemia, and pancytopenia have been reported with sulfonylureas.
  • Metformin: The most severe side effect associated with metformin are lactic acidosis. Other risk factors include congestive heart failure, trauma, severe dehydration, intravenous pyelography, arteriography, abdominal pain, and diarrhea etc. Evidence of non-ketotic acidosis must be watched for, and suspicious cases of blood lactate should be estimated.

Warning / Precautions:

Glimepiride: Hypoglycemia: All sulfonylureas are capable of producing severe hypoglycemia. Hypoglycemia is more likely to occur when caloric intake is insufficient after severe or prolonged exercise.

Loss of control of blood glucose: When a patient stabilized on any diabetic regimen is exposed to stress such as fever, trauma, infection, or surgery, a loss of control may occur. At such times, it may be necessary to add insulin in combination with Glimepiride or even use insulin monotherapy.

Metformin: Lactic Acidosis: Even though metformin is not associated with type A lactic acidosis. It should be given with caution to patients with risk factors for hypoxia such as sepsis, dehydration, congestive heart failure, seizures, or alcoholism.

Metformin therapy is associated with deficiency of vitamin B12 and folic acid; these two must be estimated periodically, and supplements may be given.

Contraindications: Glimepiride is contraindicated in patients with: Known Hypersensitivity to glimepiride.

  • Diabetic ketoacidosis with or without coma. This condition should be treated with insulin.
  • Metformin is contraindicated in patients with:
  • Known hypersensitivity to metformin hydrochloride. Renal disease or Renal dysfunction.
  • Congestive heart failure requiring pharmacological treatment. Acute or chronic metabolic acidosis.
  • This should be treated with insulin.

Pregnancy & Lactation: There are no adequate well-controlled studies in humans, but animal studies suggest that Glimepiride should not be used in pregnancy and lactation. Metformin is also not recommended for use in pregnancy.
