Timosol

Timosol (Timolol 0.5%) by Schazoo is a sterile eye drops, 5ml.It is an anti-glaucoma drops for ophthalmic use only. It is a sterile, crystal clear anti-glaucomatous solution for topical use. Timolol maleate is a non-selective β-adrenoreceptor antagonist belonging to the phenoloxy propanolamine class of sympatholytic drugs.

It reduces the elevated intraocular pressure (IOP), which is a major risk factor in the pathogenesis of glaucomatous visual field loss and optic nerve damage.

Package Contains: Each Timosol package contains eye drops in a 5ml plastic dropper bottle & a leaflet.

Composition: Each ml contains: Timosol

Timolol (as maleate) U.S.P. 5 mg.

Benzalkonium chloride as preservative, Boric acid, Sodium borate, and WFI.

Indication / Uses: Timosol sterile ophthalmic solution offers an effective, safe, convenient, and reliable therapy to normalize the consistently elevated IOP in the following pathological conditions:

  • Patients with ocular hypertension,
  • Patients with chronic open-angle glaucoma, including aphakic patients with glaucoma.
  • Patients with secondary glaucoma, e.g, steroid-induced and narrow-angle closure in the opposite eye.
  • Patients with narrow-angle of iatrogenically induced narrow-angle glaucoma.
  • Patients with paediatric glaucoma inadequately controlled with other anti-glaucomatous therapy in combination with miotics, adrenaline, or oral carbonic anhydrase inhibitors.

Contraindications: Timosol sterile ophthalmic solution is contraindicated in patients with:

  • Hypersensitivity to timolol maleate or benzalkonium chloride.
  • Sinus bradycardia, AV-block, overt cardiac failure, cardiogenic shock, and diabetes mellitus.
  • Bronchospasm, including bronchial asthma or severe chronic obstructive pulmonary disease.

Dosage & Administration: Usually, one to two drops of Timosol ophthalmic solution may be instilled in the eye(s) twice a day, but once the IOP is effectively controlled, the dosage may be reduced to one drop once a day.

Switching Over To Timosol: Add one drop of Timosol twice a day to the usual daily routine of the drug(s) already in use. On the next day, use only Timosol twice a day. When shifting from another topical ophthalmic ẞ-adrenergic blocking agent, that agent should be discontinued on one day and the treatment should be switched over to one drop of Timosol twice a day on the following day.

Children: One drop of Timosol every 12 hours in combination with other anti-glaucoma drugs.

Nursing Mothers: Because of the potential for serious adverse reactions from Timolol maleate in nursing infants, a decision should be made whether to continue nursing or to discontinue the drug, taking into consideration the importance of the therapy to mothers.

Side Effects: Timosol is generally well tolerated and is one of the safest drug used in glaucoma; however, the clinical trials have reported the following adverse effects;

  • Ocular irritation (conjunctivitis, blepharitis, keratitis), decreased corneal sensitivity and visual disturbances, including refractory changes due to withdrawal of miotics, diplopia and ptosis.
  • Aggravate certain cardio-pulmonary or other complications related to ẞ-blockers.
  • Rarely, cerebrovascular ischaemia and cerebrovascular accidents have been reported.
  • Other side effects include headache, nausea, dizziness, depression, fatigue, rash and urticaria.

Precautions/ Warnings: Timolol maleate may be absorbed into the bloodstream hence, it should be used with caution in patients with contraindications to the systemic use of β-blockers and patients with a history of pulmonary obstruction.

  • Timolol maleate may potentiate the effects of β-blockers in patients receiving β-adrenergic blocking agents either on IOP or on β-blockade, therefore a proper monitoring of the dosage is necessary.
  • When Timosol is used to reduce the elevated IOP in angle-closure glaucoma, it should be prescribed in combination with miotics.
  • In rare cases, with continuous use of Timosol, dryness of the eye(s) may occur. In such conditions, the drug should be discontinued and all β-blockers should be withdrawn gradually.
  • The use of Timolol maleate is not recommended in premature infants and neonates.
  • The use of Timolol in pregnancy has not been established so far.

Drug Interactions: In monotherapy, timolol maleate has little or no effect on pupil size; mydriasis may occasionally result from concomitant therapy with epinephrine.

The potential exists for agonistic effects leading to hypotension when Timosol is administered together with an oral calcium channel blocker, β-blocker and catecholamine-depleting drugs.

DIRECTIONS: Store in a cool and dry place. Protect from heat, light & moisture. Keep all medicines out of the reach of children. Keep bottle tightly closed when not in use. Contact lenses should be removed during the treatment. Use under strict medical supervision.
