Amlocard
Amlocard 10 (Amlodipine Besylate), Pharmatec is a 30-tablet pack. Amlodipine is a calcium ion influx inhibitor of the dihydropyridine group (slow channel blockers are calcium ion antagonists) and inhibits the transmembrane influx of calcium ions into cardiac and vascular smooth muscle.
The mechanism of the antihypertensive action of Amlodipine is due to a direct relaxant effect on vascular smooth muscle. The precise mechanism by which Amlodipine relieves angina has not been fully determined, but it reduces total ischaemic burden by the following two actions.
- Reduces afterload by widening peripheral arteries, lowering the heart’s workload, and oxygen demand.
- Improves oxygen supply by widening coronary arteries, including in areas with poor blood flow (like in Prinzmetal’s or variant angina.
Package contains: Each package of Amlocard 10 contains 30 tablets of 10mg & a leaflet. It is also available in 5mg tablets.
Composition: Each tablet contains: 
Amlodipine B.P 10mg
(As Amlodipine Besylate).
Indications: It is indicated for hypertension, angina, including chronic stable and vasospastic types.
Uses: Amlocard is a medicine from the calcium channel blocker (CCB) group. It is mainly prescribed for heart and blood vessel conditions.
- Amlocard relaxes and widens blood vessels, allowing blood to flow more easily.
- This lowers blood pressure and reduces strain on the heart, helping prevent stroke, heart attack, and kidney damage.
- Used to prevent episodes of angina caused by reduced blood flow to the heart muscle.
- It helps the heart get more oxygen by improving blood supply through the coronary arteries.
- Prescribed to improve blood flow in patients with narrowed heart arteries and to reduce chest pain.
- It can help relax blood vessels in the hands and feet, reducing coldness and color changes.
Dosage & administration: As prescribed by the physician. For both hypertension and angina, the usual initial dose is 5 mg Amlodipine tablets once daily, which may be increased to a maximum dose of 10 mg depending on the individual patient’s response.
No dose adjustment of Amlodipine tablets is required upon concomitant administration of thiazide diuretics, beta blockers, and angiotensin-converting enzyme inhibitors.
Contraindications:
- Amlodipine tablets are contraindicated in patients.
- hypersensitivity to amlodipine or any of the excipients.
- Severe hypotension.
- Shock (Including cardiogenic shock).
- Obstruction of the outflow tract of the left ventricle (e.g, high-grade aortic stenosis).
Warnings and precautions: Symptomatic hypotension is possible, particularly in patients with severe aortic stenosis. However, acute hypotension is unlikely.
Worsening angina and acute myocardial infarction can develop after starting or increasing the dose of amlodipine, particularly in patients with severe obstructive coronary artery disease. it is extensively metabolized by the liver, and the plasma elimination half-life is 56 hours in patients with impaired hepatic function. Titrate slowly when administering amlodipine to patients with severe hepatic impairment.
Side Effects: Common: Swelling of ankles or feet (edema) is the most common side effect. Headache, dizziness, or lightheadedness, Nausea, abdominal pain, or upset stomach. Fatigue or unusual tiredness. Flushing (warmth/redness in face), palpitations (feeling heartbeat), mild drop in blood pressure.
Less common: Muscle cramps or joint pain. Drowsiness. Sleep disturbance or insomnia. Gum swelling (gingival hyperplasia) with prolonged use. Constipation or diarrhea.
Serious Side Effects: Severe dizziness or fainting (possible very low blood pressure). Irregular heartbeat or chest pain worsening. Shortness of breath or significant swelling of legs/hands. Severe skin rash or allergic reaction (itching, swelling of lips/tongue/throat). Yellowing of the skin or eyes (jaundice, a rare liver effect).
Pregnancy and lactation: The safety of amlodipine in human pregnancy has not been established. In animal studies, reproductive toxicity was observed at high doses. Use in pregnancy is only recommended when there is no safer alternative and when the disease itself carries a greater risk for the mother and fetus.
Breastfeeding: Amlodipine has been identified in breastfed infants of treated women. The effect of amlodipine on an Infant is unknown. A decision on whether to continue/discontinue therapy with amlodipine should be made taking into account the benefit of breastfeeding to the child and the benefit of amlodipine therapy to the mother.
